There are a wide variety of anti-aging skin care creams available in the market today, to cater to the ever increasing demand for anti-aging products by the new age beauty conscious consumer. The baby boomer generation is growing and everyone wants to put their most beautiful, youthful faces and bodies forwards! This leads companies to try and exploit this mindset by releasing newer creams with taller and bigger claims into the market, almost every day. Money is splurged on expensive advertisement campaigns to convince people of the efficacy of these skin creams and cosmeceuticals, but do these anti-aging and beauty creams really work? Ok, time to hear the truth? :)

Most anti-wrinkle skin creams simply hydrate the skin, plumping the wrinkles out and making them appear  better ONLY temporarily. There are very few scientifically proven anti-aging ingredients like retinoids, alpha-hydroxy acids, peptides, anti-oxidants etc. However, most over the counter creams contain these in very low concentrations and hence are not very effective.This is so probably because they will be used by a large number of consumers with differing skin types without prescription, and they should not cause any irritation or side effects since they are for mass use and that too without supervision.

For good anti-aging effects, high concentration of these actives is ESSENTIAL and they have to be used only under a dermatologist’s supervision, as they are not totally free of adverse effects. For the right products for your skin, for treatment as well as maintainence and prevention, it is best to seek an expert dermatologist’s advice and use PRESCRIPTION products which contain ACTIVE ingredients and not just emollients, to keep your skin healthy and young!

What you must know about Acne (Pimples) on the skin.


1)  Acne can be in the form of small bumps on the skin of the face, chest, back, arms, thighs, buttocks and the scalp, which may be black called blackheads, white bumps called whiteheads, pink or red colored, pus filled, or big and hard nodules. When they heal, they leave dark marks and sometimes pit like scars on the skin which can badly disfigure the skin.

2) Pimples are a seen most commonly on the skin in teenagers, though they can continue into late twenties and thirties. They can suddenly appear during pregnancy and refuse to go thereafter. Some individuals may start getting it after middle age and in case of women it may start appearing around menopause.

3) Acne is generally related to hormone changes affecting the skin in teenage. Heredity, pregnancy, polycystic ovarian disease, some medicines, hormone abnormalities, high levels of P. acnes bacteria in the skin are some of the causes of acne. The role of food in causing acne is controversial; however, if you observe that particular food item consumption increases your acne, avoid it.

4) Evaluation of the skin by a dermatologist is necessary, especially for severe and persistent acne, to treat the underlying cause if any and prevent pigmentation and scarring.

5) Your dermatologist may prescribe creams, tablets or face washes for treatment. Scars and skin pigmentation require treatment with microdermabrasion, chemical peels, dermarollers etc. depending on the extent of the problem, with the latest being the fractional laser technology which is available in some centers.

6) Microdermabrasion also called skin polishing, is good for very mild acne scars and can be done with powder crystals or diamond crystals. It requires many sessions. Recently, advanced microdermabrasion followed by serum infusion (Hydrafacial) is available at a few centers.

7) Chemical peels can be done for mild acne scars and post acne pigmentation. They require multiple sessions of treatment. A wide range of peels are available ranging from glycolic, TCA, salicylic, retinoid, combination, lactic, arginine, yellow peel, blue peel, green peel etc.

8)  Dermarollers are small hand held instruments with very fine needles attached to them, which stimulate collagen formation when applied to the skin.This treatment requires multiple sessions and can be used for mild to moderate grade acne scars.

9)  Fractional lasers are available in 2 types: ablative and non – ablative. Some studies in the western world report better results with the ablative variety. However, there are yet not many studies on the comparison between the two on Indian skin. Though, personally, I agree with the world literature. The ablative type has 2 varieties – the fractional carbon dioxide and the fractional erbium laser. The first one may be used for deeper scars and the latter for shallower ones. Both require multiple sessions, the Erbium laser may need more. Darker Indian skin types do have a risk of post laser hyper pigmentation. Hence, correct patient selection, the practitioner’s experience with lasers and appropriate post laser skin care are the key to successful outcomes. Though, in case of moderate to severe scarring, the results cannot be 100%, but they can be satisfying and improve the skin to a great extent.

10) It is better to seek an expert dermatologist’s advise and get your skin treated in time, rather than try your own or peer recommended therapies and then seek advise after the damage has been done. Every individual’s skin is different and requires customized care.

QR 678 Hair Growth Factor Injections – How do they work and what do patients think and is Hair loss really such a trivial issue?!


Hair loss is impacting job opportunities, relationships, mental well-being and self confidence. It is a disease that needs treatment. Lots written on the QR 678 these past few days – Everyone seems to have an opinion on it! I am writing in to tell our part of the story on how the QR 678 works and how it impacts lives.

There is a lot of ongoing research world over to develop a wonder drug to treat this problem of hair loss, though treatments from medications to surgery are currently available. However, not many benefit, sometimes they just don’t work, there are a large number of unsatisfactory results, some patients are not suitable for available therapies, cost is huge for surgeries, results are not long lasting, side effects are a concern, etc.

What exactly is a growth factor?

A growth factor is a naturally occurring substance capable of stimulating cellular growth, proliferation and cellular differentiation. Usually it is a protein which is present locally and causes growth of that particular tissue. Growth factors are important for regulating a variety of cellular processes & act by binding to specific receptors on the surface of their target cells.

Why is this named QR 678?

A QR Code is a code used in medicine derived from “Quick Response“. 678 signifies “there is no answer”. This molecule has been named QR 678 to signify a “Quick Response to a disease which earlier had no answer”!

How did all this happen?

Stage I:

The concept originated from a chance observation by my colleague Dr. Debraj Shome, during his research trials on mice models for treatment of a severe form of eye cancer called Retinoblastoma. This research was being conducted in collaboration with the IIT Mumbai (2006), to develop a new nano-molecule drug delivery system for Retinoblastoma. This drug delivery system is injected directly around the diseased eye, so that the side effects due to intravenous delivery of the drug could be avoided. A few days after the injections were given around the eyes of each mouse, my colleague Dr. Shome observed that the hair around the mouse’s eye started disappearing due to local contact with the drug. This led him to speculate that there must be some local factors which are essential for hair growth and which were being affected by Carboplatin, since there was no other way Carboplatin could affect the hair growth, since the drug is not absorbed into the body post injection around the eye.

This was interesting as for decades, we believed that the internal health of the body was the most essential element for hair growth. This new postulate that hair growth could be influenced by local growth factors, despite the internal health of the body being perfect, was brilliant, revolutionary and new.

However, knowing that local growth factors may cause hair growth is one thing, but proving the same and finding which ONE of the hundreds of local protein growth factors actually caused hair growth or the absence of which caused hair loss, was a daunting task! These phases, undertaken in joint collaboration with a few top Indian institutes took four years of pain-staking research, & prolonged animal studies as well as human trials.

Stage II:

The next step was to identify by a process of scientific research the most likely ones of the growth factor proteins present in the scalp skin which could cause growth. By a process of scientific exclusion, we identified a total of five odd growth factors which could potentially play a role in hair growth.

Stage III:

It was not enough to merely identify the molecules though. We then prepared these five growth factor molecules artificially, outside the human body, by genetic engineering in the laboratory. These molecules were then synthesized in five different compositions and proportions, and it was decided to inject the solution into the scalp with the intention to deliver the molecules directly to the hair roots, without having to cross the skin barrier. This entire process took one and a half years.

Stage IV:

A pilot trial was then conducted with five volunteer patients in each of the five groups (n = 25), with early to moderate hair loss, being subjected to the injections. The results were the best consistently in one particular group of human volunteers. This newly created molecule thus seemed to be the most likely to cause hair growth.

Stage V:

The findings of the pilot trial needed to be validated. A randomized clinical trial was set up comparing this new molecule in different concentrations and at different treatment frequencies with a placebo injection, in 750 patients. More than 80% hair growth was seen in about 6 months for more than 90% patients who tried this injection therapy. Results in some cases are even better than hair restoration surgery. This clinical trial involved 100 patients and spanned more than a year. This trial was double-blinded and finally unequivocally proved the efficacy of this cocktail of three growth factors in causing hair growth! Thus, was born the concoction which everyone was waiting for! This molecule was named ‘The QR678’.

How does the QR 678 work?

The epidermal hair follicles are regulated by interactions between the dermis and epidermis (the 2 layers of the skin),and undergo stages of growth (anagen), involution (catagen), and silence (telogen).Factors from the hair root which is present in the dermis, act as inducting signals for this cyclic growth of the hair. There are stem cells in an area of the hair follicle which pick up these signals and form a down growth into the dermis to form the hair shaft.

The network of capillaries surrounding the base of the hair follicle delivers the nutrients – amino acids, vitamins, minerals – necessary for proper functioning. Cell division in the hair follicle is extremely fast. This rapid cell proliferation requires a constant supply of nutrients. The inducting signals for the hair follicle growth are the growth factors identified by our team in the revolutionary artificially prepared concoction ‘QR678’.

Skin Care – Golden Rules To Beautiful, Young Looking Skin!


Skin care in India, even in metros like Hyderabad, Mumbai, Delhi, etc. is offered by beauticians, cosmetologists, cosmetic companies, cosmetic dermatologists, dermatologists, skin doctors etc.No wonder with so many types of practitioners claiming expertise, the consumer is confused as to who can offer him/her the best and correct skin care solution.

1. How to diagnose your skin type -

Normal skin - Normal skin is smooth, soft, with no visible pores or blemishes, no greasy or flaky areas. It has a healthy glow.

Dry skin - Dry skin has a low level of sebum – ‘natural skin oil’, is not well hydrated and feels parched, dull and tight since it cannot retain moisture. One always feels the need to put some moisturizer or cream after washing the face.

Oily skin – The skin looks shiny, thick and dull due to overactive oil glands. It is very prone to acne and open pores. You feel the need to wash the skin repeatedly every 2-3 hours due to the greasy look.

Combination skin – The skin in the T-zone which includes the forehead, nose and the chin, is oily, and the rest of the face is drier.

2. Normal skin care -

a. Cleanse twice daily with a mild normal skin care face wash of skin pH, followed by a mild toner to keep the pores tight.

b. Apply a broad spectrum sunscreen for normal skin with a high SPF daily.

c. Apply a light moisturiser at night to keep the skin hydrated.

d. If make up is used, remove it with cleansing milk and face wash.

e. Use an exfoliating scrub two weekly followed by a non-drying face mask

3.  Dry skin care -

a. Use a hydrating creamy face wash with skin pH not more than twice daily.

b. Toners are not recommended for dry skin care.

c. After cleansing, always follow up with a skin care moisturiser containing vitamin E, Squalene, omega-3 fatty acids etc.

d. Top it with a broad spectrum high SPF sunscreen meant for dry skin.

e. Make up products if used should be cream based.

f. Weekly hydrating masks and monthly hydrating facials are recommended for very dry skin care.

4. Oily skin care -

a. Use a foaming face wash with alpha hydroxy acids, salicylic acid etc. 3-4 times a day, followed by an alcohol free toner as basics for oily skin care.

b. Moisturisers are not required.

c. Use a non comedogenic broad spectrum high SPF sunscreen for oily skin.

d. Skin care make up if used should be water based.

e. Use an exfoliating scrub weekly and anti-acne mask if suffering from acne.

f.  Seek a skin care experts’ advice if you have acne, scars and blemishes.

5. Combination skin care -

Skin care for dry and oily skin areas respectively as mentioned above.
